New Patients

How to Register

If you live within our practice area and would like to register as a patient, please complete a registration form available to download below, or pick one up from Reception.

Alternatively, sign and hand in your medical card from your previous practice.

All patients, including children, will be registered with a allocated named, accountable GP who is responsible for their overall care at the practice. This is referred to as your Named GP.  You should be informed of your Named GP when you register but you can also check who this is with any member of staff at any time.  We encourage you to see your Named GP where possible, particularly for on-going problems, so we can provide you with better continuity of care.  If he/she is fully booked, you can ask for an appointment with another Doctor or a Minor Illness Practitioner.  We will make reasonable efforts to accommodate requests to change Named GP.

Anyone wishing to register at Pilning Surgery must provide two forms of identification – one item to prove identity and one item to prove residency. All documentation provided must be current i.e. dated.

1. Proof of Identity: The person registering must provide a photographic ID. All proof must include the full name and be up to date.

We accept one of the following items:

  1. Passport
  2. Driving Licence including photograph
  3. Identification Card including photograph
  4. Visa including photograph

2. Evidence of Residency:

Evidence that you live at the address you have stated on the registration form. This must be in a typed and official format. Documents must be up to date and include your full name and full address.

We accept one of the following items:

  1. Utility Bill (including Mobile Phone)
  2. Tenancy Agreement
  3. Mortgage Statement
  4. Bank Statement
  5. Payslip
  6. Driving Licence (showing current address)
  7. Official UK Benefits paperwork i.e. Job Seekers Allowance, Income Support etc.

Please ensure you provide one item from each of the lists above.
